The simplest systems are often the ones I keep using. This one has only three parts: what happened, what I think it means, and what I’ll do next.

Start with evidence

I write down the observable result before explaining it. That small pause makes it harder to turn an assumption into a fact.

End with a decision

A note is more useful when it changes the next action. If it does not, it may still be interesting—but it is not yet operating guidance.
